It gives us great pleasure to inform you that with the supporting of the Association of Hong Kong Diabetes Nurses (AHKDN), Hong Kong Dietitians Association (HKDA), Physical Fitness Association of Hong Kong, China (PFA) and Youth Diabetes Action (YDA), the captioned Forum will be held on 5 May 2019 at the Kowloonbay International Trade and Exhibition Centre, Kowloon Bay, Kowloon, Hong Kong.
The complexity of diabetes calls for a holistic approach to address the biomedical, social, cognitive, psychological and behavioral needs of subjects at risk or with diabetes. There is now emerging evidence showing that information technology, patient empowerment and collaborative care can make real difference to clinical outcomes. In this Forum, experts in the region will gather together to share their best practices in preventing and controlling diabetes by using registry to identify unmet needs and innovative approach to rise to this global challenge.
